1만성기 뇌졸중 환자에게 발과 발목관절에 키네시오 테이핑 적용후 족저압과 보행에 미치는 영향

Purpose This study was conducted to evaluate the effects of kinesio taping applying to foot and ankle joints on foot pressure and gait in patients with chronic stroke. Methods This study was a within-subject, between-condition comparative design. We enrolled Five stroke patients with chronic stroke. Patients had Kinesio taping applied on tibialis anterior, tibialis posterior, peroneus, and foot arch. We used TPscan to measure the foot pressure and G-walk to analyze walking ability. We measured foot pressure and gait ability with and without foot and ankle taping. Result There was no significant difference in foot pressure between the foot and ankle joints (p> 0.05). Cadence, stride length, stance phase, and swing phase of gait pattern were not significant difference (p> 0.05). However, for cadence, stride length, there showed a large effect size and for stance phase, showed from moderate to large effect, and for swing phase, showed a small effect size. Conclusion The application of foot and ankle joint kinesio taping to patients with chronic stroke had no significant difference after using taping but showed from small to large effect. Therefore taping may positively influenced gait ability.

Purpose we examined the change of function and pain by improvement of the temporomandibular joint during low-level laser(LLL) and transcutaneous electrical nerve stimulation(TENS) to adults who show temporomandibular disorder. Methods This study was conducted with temporomandibular disorders. Participants were randomly divided into two group (LLL vs TENS). The TENS was applied at a frequency of 4Hz and intensity for 15 minutes. The LLL was applied with a power of 40㎷ per laser at 685nm wavelength in continuous wave mode. This study were conducted three times a week for a total of 4 weeks. Results The LLL was associated with more significant improvements in mouth opening (p<0.01, 95% CI: 1.910-7.023), and masticatory muscle pressure threshold(p<0.05; 95% CI: 0.027-0.943). Conclusion Our findings indicate that LLL was more effective than TENS in the treatment of TMD. Such findings highlight the need to develop and promote a controlled therapy program to facilitate a return to normal daily activities in patients with TMD.

Purpose The purpose of this study was to investigate the effects of facilitating paretic arm through placing training in variety space with single case report. Method The subject was elderly men aged 55. The intervention was performed for 1hour per section, 13 times in total. The outcome measure was used 2D video analysis program (Dartfish ProSuite Express) for the angle of upper limb motion segment and measuring arm reach time with video analysis. In a supine position, a subject's arm was reached vertically with being opened wirst and finger by the therapist, and then maintaining the arm in space. This process was under the assistance of a therapist. Since then, resistance and task-oriented exercises were additionally implemented. Result In a supine position, the compensatory angle of shoulder joint abduction decreased and the supination of wrist joint increased. Improving supine position also helped improve arm functional movement in a sitting position. The angle of shoulder flexion was increased, and the compensatory angle of shoulder abduction was decreased. Also, the time it took to reach subject's arms 90 degrees from supine position was significantly reduced. Conclusion This study demonstrated placing training on reaching movement showed significant improvement in function of the affected upper limb reaching movement.

Purpose The purpose of this study was to investigate the effects of sensory integration therapy combined with physical activity according to developmental stage on developmental and visual development of retarded child. Methods A total of 20 sessions of sensory integration program were performed for 9 weeks, three times a week for 40 minutes, 30 minutes for sensory integration therapy and 10 minutes for fine motor activities. Fine motor activity was performed 1 : 1 with the therapist. Preliminary and post-test were performed with the Short Bruininks-Oseretsky test of Motor Proficiency(Short BOT2) and the Korean Development Test of Visual Perception(K-DTVP-2) for the dependent variable measurement. Results The results of the study showed that the scores of the children's exercise and visual perception were improved. Based on the results of this study, it is expected that sensory integration therapy combined with physical activity according to the developmental stages applied to infants with developmental delay promotes overall development and helps positive development of exercise and visual perception.

Purpose The purpose of this study was to examine the influence of repeated vibration stimulus on wrist and elbow flexor muscle of chronic stroke patient to hand dexterity, grip strength and activities of daily living. Methods The 23 subjects were participated in this study and randomly divided to two groups: vibration group, non-vibration group. Vibration group received vibration stimulus on wrist and elbow flexor muscle 5times a week for 2 weeks and to examine its effectiveness, and study conducted re-evaluation 2 weeks after intervention. 23 participants were measure using Fugl Meyer Assessment(FMA), Box and Block Test(BBT), JAMAR hand dynamometer, Modified Barthel Index (MBI). Results Vibration group and non-vibration group were significantly difference FMA total, BBT, JAMAR hand dynamometer(p<.05). Only vibration group was significantly difference between pre and post in FMA wrist(p<.05). Also, when comparing the changes value between the group pre and post BBT shower significantly difference(p<.05), and comparing the changes value between the group pre and 2 weeks after intervention JAMAR hand dynamometer showed significantly difference(p< .05). But, MBI was not significantly difference(p >.05) Conclusion Repeated vibration stimulus on wrist and elbow flexor muscle of chronic stroke patient was improved hand dexterity, grip strength and furthermore, grip strength showed effectiveness after intervention.

Purpose The purpose of this study is to analyze the functional improvement through task-oriented training of stroke patients and to suggest the clinical application effect of task-oriented training through systematic review. Methods From the period of 2011 to 2016, we searched the papers which received the original text downloading service among domestic journals and theses published in e-journals. Major search terms were “task-oriented” by searching “stroke” A total of 108 papers were searched by using appropriate combinations of the words “upper extremity”, “lower extremity”, “gait”, “mental”, “sense” and “Seventeen papers were selected for inclusion and exclusion criteria. Results A total of 17 studies were classified according to body function and task performance included in the framework of occupational therapy execution system. The results were as follows: 10 studies (58.8%) on neuromuscular movement, 2 (11.8%), and 5 (29.4%) were related to daily life activities. The results of the study showed that task-oriented training was effective in improving the function of stroke patients. Conclusion Task-oriented training showed a positive change in functional recovery of stroke patients, suggesting that task-oriented training is needed for stroke patients. Therefore, in future research, it is necessary to study the task-oriented training development of stroke patients. Based on this, we will give how clinicians help occupational performance and body functions through more systemic review, and will be able to suggest evidences future intervention direction for patients following intervention effect.

Purpose The purpose of this to investigate the effect of hand movement training based on virtual reality on the upper extremity and hand function of stroke patients and to present a therapeutic evidence for virtual reality training intervention. Methods Subjects were randomly assigned to two groups of 30 stroke patients, 15 experimental subjects and 15 control subjects. Experimental group performed hand movement exercise based on virtual reality, and control group performed general occupational therapy. We performed FMA-UE, WMFT, and BBT evaluations to changes in upper extremity functions. Results The experimental group participating in the virtual reality training showed statistically significant improvement(*P<.05) in the FMA-UE and WMFT evaluation in order to investigate the change of upper extremity and hand function, A statistically significant improvement(**P< .01) was also observed in the BBT evaluation of the hand agility to show that the upper limb function improved after the virtual reality training intervention. There was no statistically significant difference between the two groups(P >.05), but there was a statistically significant difference in the amount of change in upper extremity function between the two groups (*P<.05). Conclusion We found that hand movement training based on virtual reality has a positive effect on recovery of upper extremity and hand function in stroke patients, We hope to discuss various issues to be solved in the approach of upper limb treatment for stroke patients based on virtual reality in the future.

Purpose The purpose of this study was to investigate the effects of sensory stimulation and oral motor exercise program on oral feeding and development in premature infants. Methods Eight of the experimental group had sensory stimulation and oral motor exercise and 7 control subjects who performed oral motor exercise alone. There are 15 people in total. They were randomly assigned to two groups according to the order in which they visited, through a random number table using a computer program. The mediation period was 5 days for 4 weeks, 20 minutes for the experimental group and 15 minutes for the control group. Results In the comparison of pre - and post - intervention effects between the two groups, amount of oral feeding, Weight, Neonatal Oral Motor Assessment Scale (NOMAS) jaw, and tongue grade showed significant changes in both groups. Comparisons of pre - and post - intervention changes between the two groups showed significant changes in the amount of oral feeding in the experimental group. Conclusion Both the experimental group with sensory stimulation and oral motor exercise and the control group with oral motor exercise program showed positive effects on oral feeding and development. The effects of oral motor exercise program and sensory stimulation intervention were more effective in increasing oral feeding than oral motor exercise alone.
